LIFEcard Health™ Privacy Policy

Emergency 24, Inc., (“EM24”) values the privacy of our LIFEcard Health Members and Card Holders and takes precautions to safeguard their personal data and health-related information from unauthorized use. The following is a detailed explanation of our privacy policy with regard to the LIFEcard Health web site and our Services.

EM24 offers secure database services to our Members and to any other individual(s) (Card Holders). EM24 does not lease, license, sell or share any Member or Card Holder personal data or health-related information with anyone or with any company that is not authorized under the terms and conditions of the Services requested by the Member or Card Holder. EM24 does not capture any credit card information, all transactions go through Verisign, one of the largest transaction processors in the Internet.

Member(s) and other individual(s) have the ability to access and to maintain their account data and information via the Internet. Some of the information gathered and available through this service includes: 1) general Card Holder information: name, address, email addresses, phone numbers, password; 2) physical features of the Card Holder: date of birth, gender, height, weight, color of eyes, blood type, corrective lenses; 3) doctor contact information: name, specialty, address, phone number; 4) general emergency contact information: names, relationships, phone numbers, email addresses; 5) health conditions: condition, description, 6) medications: name, description, dosage, frequency, strength, notes; 7) insurance information: company name; policy number, group number, dates, phone numbers, address, notes. As a user of our web site and/or a participant in our LIFEcard Health Service, you hereby acknowledge that certain personally identifiable information concerning you is collected and stored by EM24 and may be viewed by employees of EM24 in order to properly perform the services that you are requesting. All EM24 employees have signed confidentiality agreements, and their backgrounds have been checked through state and federal agencies.

Other people who are authorized and may view or access your personal data and information are those who have direct access to your account number (Card No.) and your security code (Code), which is on the LIFEcard Health Card that the Card Holder is issued. It is solely the Member’s or Card Holder’s responsibility to secure and safeguard the LIFEcard Health Card information from unauthorized use. People who have access to Member’s or Card Holder’s Login “Username” and “Password” have access to the personal data and information and they have the ability to “modify” any and all data in the account. It is solely the Member’s and Card Holder’s responsibility to secure and safeguard the Member’s or Cardholder’s Login “Username” and “Password” from unauthorized use. If a third-party individual or entity accesses the Card Holder’s personal data and health-related information through “authorized” channels, EM24 is NOT responsible for how that third-party individual or entity secures and safeguards the Card Holder’s personal data and health-related information.

Another time someone other than an EM24 employee or someone who has access to your account number (Card No.) and your security code (Code) which is on the LIFEcard Health Card will be authorized to see the Card Holder’s personal data and health-related information is in the case of a judicial request (such as a subpoena) or if EM24 or part of EM24 is acquired or sold. In the event that the latter happens, EM24 will give reasonable notice to all existing Members and/or Card Holders of its LIFEcard Health Services.

If you lose your card, notify EM24 as soon as possible. EM24 will transfer your personal data and health-related information to a new account number and issue a new LIFEcard Health Card. You will be responsible to pay the prevailing rate charge for the new card.

EM24 reserves the right to change this privacy policy at any time and will notify you accordingly if any such changes take place.
