A living will is a document which
enables you to set guidelines for
dealing with life-sustaining medical
practices and procedures. In the
event of sudden debilitation, living
wills can inform medical personnel
of your personal health requests,
such as the donation of organs or
your preferences when it comes to
life-sustaining or life-preserving
medical procedures. If you ever
become incapable of expressing
yourself or are suffering from an
incurable and terminal condition,
these documents can appoint someone
to make important health care
decisions on your behalf in case you
are unable to do so.

Prepare your customized living will
by following the instructions below.
You must be a registered LIFEcard
Health member to store your living
will within the LIFEcardHealth.com
database.

to access an appropriate, state
specific living will.
- Read the forms carefully and
complete the necessary fields.
- Write your LIFEcard Health card number at the
top of the living will.
- Fax the document to LIFEcardHealth
- 773-286-1992
- LIFEcard will append your account
with your living will.
